ACCIDENT INVESTIGATION PROCEDURE

Accident investigation guidelines differ depending on how severe the accident is. The following are some of the guidelines used to investigate accidents.
  1. Obtain general information This involves asking things question like if anything has been changed since the accident occured, names of injured employees, eye witnesses, condition of any equipment taking photos of any equipment and the time, layout and area.
  2. Obtain witness statement. This involves the name, contact and occupation of the witness, what they witnessed and what they did.
  3. Establish circumstances. What was being done at the time and what happened, if something similar has ever happened previously, what instruction and training was given before and many other question.
  4. Immediate response to accident. What immediate action was taken when the accident occured.That is was first aid given?,was there any fire fighting?
  5. Identify prevent measures. Was there up to date risk analysis or assessment carried out for the task?Review risk assessment for the report.
  6. Identify underlying causes. Was supervision and training adequate? Was the equipment suitable for the task?Was communication adequate for relevant between parties?
  7. Actions to prevent a recurrence. What need to be done to prevent the occurence of such an accident.Were the safety precautions adequate but not implemented?Actions to prevent recurrence of accidents include: better guarding or barriers, better test and maintainance schedules, better communication, improved supervision, training, instruction and information.
  8. Post accident risk assessment. The results of the investigation will show if existing control measures are adequate or what remedial action is needed.
  9. Report the recommendations. Report what actions are necessary to prevent and eliminate the hazards or control the risks the hazards pose.This will help an organization to prevent future accidents from occuring.
